ATM Card
Case 1 - First time application
  • Apply for the card : ATM card - application form
    • Card PIN will be issued automatically to corresponding address;
    • Card needs to be collected in person;
  • Authorize another person to collect card : ATM card collection authorization form
  • Please send signed forms to your banker for processing;
  • Please send signed authorization letter to authorized person;
Case 2 - Non-delivery of applied card
  • Apply for the card : ATM card - application form
    • Please mark "non-dilvery of card" in Section A - Others;
    • Card PIN will be issued automatically to corresponding address;
    • Card needs to be collected in person;
  • Authorize another person to collect card : ATM card collection authorization form
  • Please send signed forms to your banker for processing;
  • Please send signed authorization letter to authorized person;
Case 3 - Apply for PIN only (already have ATM card)
  • Apply for lost card PIN : ATM card - PIN enquiry
    • Card PIN will be issued automatically to corresponding address;
  • Please send signed forms to your banker for processing;
 
 
  • Client can authorize us to collect the card.
    Please just sign authorization form and send to us by mail or courier.
  • Usually, the bank will keep the card for around 1 month.
    The card will be cancelled if not collected. Client may need to apply again when he is available to pick up.
