  • The form : Change of Address and/or Customer Information Instructions Form
    1. Write new address in Page 1 - "Address" section;
    2. Select "Corresponding Address" and also fill out the corresponding address;
    3. Specify account numbers and credit card number (if any) on Page 2;
    4. Sign by authorized signature on Page 2;
    5. Send original back to the address stated on Page 1;

  • The form : Chinese Correspondence Address Registration Form
    1. Write new address in Page 1 - "Address" section;
    2. Fill our the corresponding address in Chinese characters;
    3. Sign by authorized signature on Page 2;
